Notice of Eviction / Termination of Tenancy
You are hereby notified that your tenancy at the above-described premises is being terminated and that you are required to vacate the premises for the reason(s) stated below. This notice is given pursuant to the Oklahoma Uniform Residential Landlord and Tenant Act (URLTA), Code of Oklahoma §35-9A-101 et seq.

Cure Period and Payment Instructions (For Nonpayment or Curable Violations)
For nonpayment of rent:

- Partial payment does not cure the default unless the Landlord agrees in writing to accept less than the full amount owed.
For curable lease violations (other than nonpayment):

- If the violation is cured within the seven-day period, this notice is withdrawn as to that violation.
- If the violation is not cured, you must vacate the premises at the end of the seven-day period.
Delivery of This Notice
This notice is being delivered by one or more of the following methods permitted under Oklahoma law:
- Personal delivery to the Tenant or to an adult member of the Tenant's household at the premises.
- Posting a copy of the notice in a conspicuous place on the premises AND sending a copy by first-class mail to the Tenant's last known address (which may be the premises address).
The date of service is the date of personal delivery or the date of posting (if posting + mail method is used). The Landlord or agent will retain proof of delivery.
Tenant Rights and Landlord Remedies
After service of this notice and expiration of any applicable cure or notice period, if you remain in possession, the Landlord may file an unlawful detainer action (eviction lawsuit) in the appropriate District or Circuit Court of Oklahoma to recover possession of the premises.
- In an unlawful detainer action, the court may award possession to the Landlord, unpaid rent, damages, court costs, and reasonable attorney fees if provided by the lease or statute.
- You have the right to appear in court and present any legal defenses you may have.
- Self-help evictions (changing locks, shutting off utilities, removing property without court order) are not permitted under Oklahoma law.
- If you believe this notice is defective or that you have a valid defense (such as improper service, retaliation, discrimination, or that rent was properly paid or tendered), you should seek legal advice immediately and appear in any court proceeding.
Move-Out Requirements
If you vacate as required:
- Remove all personal property and return all keys, access devices, and garage door openers to the Landlord.
- Leave the premises in a clean and undamaged condition, reasonable wear and tear excepted.
- Provide a written forwarding address to the Landlord for return of any security deposit (subject to itemized deductions permitted by law and the lease).
- Settle any outstanding rent, late fees, or other amounts owed.
Failure to vacate may result in court action and additional costs.
